The objective of this programme is to provide continuum of care for clients / families having completed Primary Treatment. This programme includes facilitated group therapy sessions for 52 weeks, regular one-to-one review meetings, on-going education and support groups.

A Continuum of Care Coordinator assists clients together with ensuring the smooth running of this level of rehabilitation. Clients as part of their Continuum of Care Plan may be eligible to take a place in one of our transitional houses in which case s/he will be in the direct care of the Transitional Coordinator.
